Long Distance Ordering Procedure

Service Delivery Division has contracted with Qwest Communications for long distance services. Please note that in addition to State agencies, Cooperative Purchasing members are also eligible for services under this contract. The rate is $.031 per minute for all calls including calls made with a Qwest calling card. For more detailed pricing information please refer to Exhibit 4 on page 15 of the contract at:

The Procedure

To place an order for long distance service using the state of Ohio Qwest contract, please fax the following information to Kathy Milligan at Qwest Communications on 1-888-666-9003.

Information needed:

  1. State of Ohio contact name and number for Qwest installation and repair
  2. Billing information:
    • Name
    • Address
    • Billed Telephone Number and Working Telephone Number
    • Type of media required for invoice: paper or CD
  3. Hours of operation
  4. Outbound service:
    • The phone numbers of the lines that will need access to long distance services
    • Primary Interexchange Carrier (PIC) information:
      • Qwest or customer to PIC the long distance carrier
      • Specify whether IntraLATA as well as InterLATA calls should be PICd.
  5. Inbound service:
    • The number of lines needed
    • Is this for 800 Service?
  6. Location of the point of demarcation (where the phone service enters the building)
  7. Calling cards: If calling cards are needed, restrictions required and how many cards
  8. The name and number of a technical contact for follow-up questions by Qwest
